Please wear or bring comfortable loose-fitting clothes. Have a least a light meal 2-3 hours prior to your appointment. We may examine your tongue to evaluate your condition, so please avoid brushing or scraping it for at least 48 hours prior to your appointment. You should also avoid food and drink that can discolor the tongue.

If the appointment is an initial one, please complete and bring the Health History Questionnaire. You may also wish to bring information about your condition such as diagnostic imaging (x-ray, MRI, CT, ultrasound), lab test results and diagnostic reports.

Please also bring an acceptable form of payment as payment is expected at the time of your appointment. Please bring a valid credit card because we require valid credit card information for our files as part of our Patient Financial Agreement. If you plan to pay with insurance and your insurance coverage has been confirmed, please complete and bring the Assignment of Insurance Benefits.
